Dancers/Parents must attest that:
I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
I have not traveled internationally within the last 14 days.
I have not traveled on an airline or visited a highly impacted area within the United States of America in the last 14 days.
I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non- contagious by state or local public health authorities.
I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold On Your Toes harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from OYT. This liability waiver and release extends to the studio together with all employees.
The Following Guidelines/Rules Have Been Put in Place at OYT:
Dancers must bring and wear masks while in the studio, including during class. “Mask breaks” will be allowed when dancers are in the studio for longer than one class. Studio or staff will give dancers direction on when “breaks” are possible and where dancers can safely take their masks off.
Only dancers and staff will be allowed into the studio. Pick up and drop-off instructions will be sent out separately.
OYT may conduct no contact temperature checks.
Dancers will be required to use hand sanitizer upon entering and exiting the studio.
Shared props will not be used.
Dancers will be given spacing directions by the instructor and they must adhere to these rules.
The lobby/waiting area is closed until further notice.
Only one dancer should be in the restroom at a time. Young dancers that require help are the exception.
Dancers should follow CDC guidelines for hand washing (20 seconds with warm water and soap).
It is prohibited to provide water from the bathroom or the bubbler per Massachusetts State Law. Dancers are required to bring their own water. NO EXCEPTIONS.
Students may be required to provide therabands, light weights, and a yoga mat or large towel. You will be notified prior to class.
All commonly touched areas (i.e. handles, switches, etc.) will be cleaned regularly.
